I tie the wii remote to my lower leg with a soft bathrobe belt so that it is comfortable and not too tight. Then I set up my chair and cycle (http://www.walmart.com/ip/Gold-s-Gym-Up ... e/11030094) infront of my wii and select a running program. It's not as fun as steering around the island on a bike ride but it's much easier on my knees and I still get a good cardio workout. I really wish they would change the program so a nunchuck in a pocket on a leg registers those movements and we could also steer with a remote in hand. Then someone could use any equipment they have, be it an exercise bike, treadmill, trampoline, etc.
